Aesthetic dissatisfaction
A bump that came back, asymmetry, or a result that was over- or under-corrected and never quite settled.
A second-chance nose surgery in İstanbul by an ENT surgeon — correcting how your nose looks and, just as importantly, how it breathes after a previous operation.
Revision rhinoplasty — sometimes called secondary rhinoplasty — is surgery to correct or improve the result of a previous nose operation. The concern may be how the nose looks, how it breathes, or both at once.
It is generally more demanding than a first rhinoplasty. Scar tissue from the earlier surgery, altered anatomy and — often — a shortage of cartilage to work with all make the procedure more delicate and more dependent on the surgeon’s experience. This is not the place to choose by price alone.
In İstanbul, Op. Dr. Arda Pelen approaches revision as an ear-nose-throat (ENT) surgeon. That matters here more than almost anywhere: a great many people seeking revision were left with a breathing problem by an aesthetic-only first surgery — a collapsed valve, a septum that was never addressed, an over-resected bridge. Because the airway and the appearance are assessed and rebuilt together, the aim is a nose that finally both looks and works the way it should.
The goal is honest and specific: a meaningful, natural improvement on what you have now — not a promise of a perfect, brand-new nose. What is realistic for your particular case is something we confirm only after a careful assessment.
You may be a candidate for revision rhinoplasty if you are unhappy with the result of a previous nose operation — or struggling to breathe through it — at least around twelve months have passed since that surgery, and you are in good general health with clear, realistic goals.
A revision works through scar tissue and altered anatomy, often with little cartilage left to reshape. That usually calls for an open approach for full visibility, and frequently for cartilage grafts to rebuild structure and support.
When cartilage is needed, the gentlest source is used first: the septum, then the ear, and only where a major rebuild is required, a small amount of rib cartilage. Just as important is the airway. Many revision cases exist precisely because a first, appearance-only surgery weakened the breathing — so the septum and the internal valves are assessed and restored as part of the same plan, not left for later.
Cartilage from the septum, ear or rib is used to rebuild support where an earlier surgery removed too much.
Collapsed valves and an untreated or weakened septum are corrected, so breathing is rebuilt along with the shape.
How air actually moves through your nose — assessed, not assumed.
The internal wall that supports both how the nose works and how it holds its shape.
The framework that cosmetic-only approaches often leave untouched.
Aesthetics designed onto a nose that already works — never the other way round.
Each pair is shown at a consistent angle and lighting, with consent on file. Revision outcomes vary more than primary surgery from one person to the next — these are examples of what is possible, not a guarantee of your own result.

Most revisions fall into a handful of patterns — and very often two or three appear together. A careful assessment identifies exactly which apply to your nose before any plan is made.
A bump that came back, asymmetry, or a result that was over- or under-corrected and never quite settled.
A blocked nose since the first operation — often a collapsed valve or a septum that was never treated. Septum surgery →
A tip that was over-narrowed, leaving it pinched, asymmetric or unsupported, sometimes with breathing affected too.
Fullness or rounding just above the tip that develops after surgery, blunting the profile line.
A nose that healed crooked or off-centre, or where one side differs noticeably from the other.
Over-resection or a saddle deformity where support has been lost and structure must be rebuilt with grafts.
As a rule, wait at least twelve months after your previous rhinoplasty before having a revision. The nose needs that time for swelling to fully settle and tissues to soften, so that the true result — and what genuinely needs correcting — is clear.
Operating too soon, into tissue that is still healing and inflamed, makes delicate work harder and the outcome less predictable. For more complex cases the wait can be longer. The one exception is a clear functional problem — significant difficulty breathing — which is assessed on its own timeline. If it is better for you to wait, we will say so plainly.
Revision results in everyday life, once the longer swelling of a second operation has fully resolved. Every result is individual — these are representative examples, not a guarantee of your outcome.

Revision rhinoplasty is performed under general anaesthesia and usually takes 2–4 hours — longer than a first operation, because scar tissue must be released and structure often rebuilt with cartilage grafts before the nose is reshaped and supported with a light splint.
Photos, examination and any previous records to understand exactly what was done.
A video consultation to agree realistic goals, the technique and any grafts needed.
2–4 hours under general anaesthesia at an accredited hospital, then one night’s stay.
Cast removed around day 6–8 before you fly home, then long-term remote check-ins.
Revision rhinoplasty is a well-established operation, but because it works through scarred, altered tissue it is more demanding than a first surgery — which is exactly why surgeon experience and an honest assessment matter so much. The risks are explained fully before you decide.
Most people return to everyday life within 7–14 days, once the cast is off and the early bruising fades. Because a revision involves more work, swelling settles more slowly — and the final, refined result appears over 12–18 months.
Mild discomfort, congestion and swelling are normal. You rest; the team stays in touch.
The external cast comes off — usually just before you travel home. First glimpse of the new shape.
Most bruising has faded; many return to work and light routines. Strenuous exercise still paused.
Deeper swelling subsides week by week; thicker or previously operated skin takes a little longer.
Revision swelling is the slowest to resolve; the definitive, settled result appears over a year or more.
Good aftercare matters even more in a revision. The essentials are simple: rest with your head elevated, keep the area — and any graft donor site — clean and protected, avoid anything that raises pressure in the nose, and follow your surgeon’s guidance closely.
İstanbul is one of the world’s leading destinations for complex nose surgery — for high-volume, experienced surgeons, accredited hospitals and expert medical care at a fraction of UK or EU prices. For a revision in particular, the experience of the surgeon, not the size of the package, is what counts.
Revision needs a surgeon who lives in the nose — operating on damaged structure and breathing together, not appearance alone.

I had my surgery in May of 2023, so it’s now been 1.5 years as I’m writing this. I was very impressed with the overall experience. I chose Dr. Pelen based on his Instagram and reviews online — and it felt very safe that he operated at Memorial Bahçelievler, one of the best private hospitals. On the day of the operation I met with Dr. Pelen to discuss what I wanted; he was honest with his thoughts, and ultimately I trusted him to do what he thought best, as he is the expert. I’m super happy with the results and finally don’t feel the need to hide my nose from the worst angles. It now suits my face and has enhanced my other features. 100% recommend Dr. Pelen.
I had a rhinoplasty 1 year ago because I had a bump on my nose, and I admit that at the time it changed my life! I’ve gained much more confidence in myself, and for people who have small modifications to make, I recommend it 100% — the result is so natural.
I had an amazing experience from start to finish with Dr. Arda Pelen and his team — all extremely professional and accommodating. Dr. Pelen answered all my questions and put my mind at ease. His bedside manner, intelligence and professionalism are out of this world. I told him what I wanted and he gave me exactly that. I’ll recommend anyone to Dr. Pelen. Thank you so much!
I chose Dr. Arda after seeing amazing results from two friends who had a rhinoplasty with him. I had no doubt about his work, but my results exceeded my own expectations. His talent is more than a nose job — he gives you the nose that suits your facial features and looks perfect for you as an individual. Flawless, natural and super flattering. He’s really a master of his craft. The experience was wonderful, with kind, helpful staff throughout. No anxiety, no struggle. Very professional and reliable.
I researched for about 2 years to find the most suitable doctor for my rhinoplasty, and I came across Dr. Arda Pelen. His work is amazing. He and his assistant guided me throughout the whole journey and made sure my experience was great. He listened to all my concerns, was very honest about the outcome, and advised what he thought suitable for my face. The nurses took great care of me too. It’s been one month and I’m already so happy with my results — nicely lifted and curved.
Amazing experience, good care, and I loved the assistants too. They were so helpful and fast with every question we needed answered. Thank you so much, Dr. Pelen — you did all of us well. We’re more than satisfied with the results.
I recommend Dr. Arda. He’s so nice, honest and so good at his job. I had my surgery 4 months ago and couldn’t wish for better results than this! He’s awesome — I’ve received so many compliments since.
I’m so happy I got my nose done by Dr. Arda Pelen! My nose looks amazing and I’m just 6 weeks post-op. I recommend him to everyone who wants a smaller, cute nose. The service in the hospital was perfect and my healing was really good, without any complications.
I’m grateful I found Dr. Galip Arda Pelen, because he fulfilled my dream of a beautiful nose with a perfect shape. The transformation is pure art — I’m very happy with the result and didn’t expect it to come out so well. He’s kind and understanding about respecting the patient’s wishes, and the hospital was amazing; the people there are wonderful and helped me relax.
Proud and thankful for Dr. G. Arda Pelen. I’d anticipated a rhinoplasty for many years. After a long wait and thorough research to find a doctor who would bring my vision to life, I found his profile. He communicated with me at the consultation, before surgery and at my follow-up in English, and listened to all my concerns. He even asked whether I’d had any trauma to my nose in previous years, which I’d forgotten to mention — I was grateful for his knowledge and insight. Honestly, I’m not sure mere words can do him justice.
I'm so glad I chose Arda Pelen. My priority was an ENT surgeon, and I researched many. The process was painless — no bruising, no breathing problems. Three weeks in, I sleep and breathe better than ever.
I just had my nose surgery and I'm very happy with the results. Dr. Arda Pelen did a wonderful job — he and his team are genuinely kind.
Dr. Arda performed my rhinoplasty eight years ago — natural, aesthetic results and real care throughout. Years later we chose him for my sister too, without hesitation. A doctor you can trust completely.
After a failed previous nose surgery I had serious breathing problems. With his revision rhinoplasty I got a result I love — and my breathing was completely fixed. Thank you, Dr. Arda.
I had my surgery five months ago — light bruising, no pain, and a nose even more beautiful than I imagined. Thank you to him and his assistants.
I can't stop looking in the mirror. Despite having panic attacks, I got through the whole process so easily — entirely thanks to Dr. Arda. When people ask who my doctor is, I proudly say Arda Pelen.
The right side of my nose was blocked — it was opened and I had aesthetic surgery at the same time. I'm extremely happy with the process, my breathing and the look.
An incredible doctor — he operated on my mum, my sibling, me, even my friend. He doesn't disappear after surgery; no matter how much time passes, you get the same care. Don't even look for anyone else.
It turned out beautifully, and they helped with every question afterwards. So glad I had my surgery with Dr. Arda — years have passed and I'm still delighted.
Dr. Arda is reassuring, communicates well, and stays in touch long after the process. His assistants are just as warm and professional.
